Health Risk Assessment (HRA)
The HRA is a self-reported questionnaire completed by participants that assess their current health and lifestyle behaviors. HRA responses are compiled into a participant report and an aggregate report. Questionnaires are available in English and Spanish.
Wellness/Lifestyle Program
Interactive educational program guided by HMC Health Coaches online or telephonically to achieve realistic collaborative goals. Options include Weight Management Support, Activity & Exercise Tips, Healthy Eating Guidance, Stress Management Assistance, Smoking Cessation Support, Heart Health & Disease Prevention.
Health Management & Education Program
Identification and stratification via our robust analytic tools provide our nurses and/or health educators with guidance to assist participants with their chronic condition(s) as well as offering treatment compliance, closing gaps in care, and medication management.
Analytic Reporting Tools
HMC provides flexibility to define and generate reports that is preconfigured to perform the DMAA standard methodology. We can also provide savings and program performance that can be run on a net paid claims, total allowed amount or utilization basis.
